ABSTRACT

Objective:To explore the association between weight change from age 20 to middle age and diabetes risk. Methods:A total of 10 104 residents over 40 years old were recruited in Guangzhou. The final analysis data set is a cohort of 6 272 patients from the REACTION study. Quartile of weight changes from age 20 to middle-age was used as an independent variable to explore its association with newly diagnosed diabetes. Simple and multivariable logistic regression analysis were used to calculate OR and 95% CI. Results:The prevalence of newly diagnosed diabetes in study cohort was 15.7%. Weight changes from age 20 to middle-age was significantly higher in diabetes group( P<0.001). Logistic regression analysis found in general population and overweight people [24.0≤body mass index(BMI)<28.0 kg/m 2], after adjusting for gender, age, educational level, family history of diabetes, drinking, SBP, waist circumference, cholesterol, high-density lipoprotein-cholesterol(HDL-C), TG, ALT, AST, the risk of diabetes increased in line with the magnitude of weight change( P=0.081). While in group of lean, normal and obese adult, no significant association has been found. Conclusion:The risk of diabetes increased with weight change in general population and overweight people, indicating that weight change in adults after age of 20 was positively associated with diabetes. It is suggested the importance of weight management in the prevention of dialetes.

ABSTRACT

Objective To investigate the correlation between hepatic triglyceride content and glucose lipid metabolism, insulin resistance and β cell function. Methods A total of 32 type 2 diabetic patients with nonalcoholic fatty liver disease was recruited in this study.Hepatic triglyceride content was measured with liver proton magnetic resonance spectroscopy.Oral glucose tolerance test (OGTT) was carried out in all participants, with measurements of plasma glucose and insulin levels. The homeostasis model assessment insulin resistance index (HOMA-IR),hepatic insulin resistance (HIR),and Matsuda Index (MSI) were used to assess insulin resistance. The homeostasis model assessment beta cell function (HOMA-βF),early insulin secretion index (EISI) and late insulin secretion index (LISI) were used to assess β cell function. Results Hepatic triglyceride contents had positive correlations with body mass index (BMI),waist circumference,Body fat, aspartate transaminase (AST),alanine transaminase (ALT),triglycerides (TG),HOMA-IR,HIR,and negative correlations with MSI.Stepwise regression analysis showed that body fat and HOMA-IR were independently risk factors for hepatic triglyceride contents. Conclusions Hepatic triglyceride content is closely correlated with obesity, liver function, blood lipid, and insulin resistance;especially obesity and insulin resistance are the most important factors.

ABSTRACT

[Objective]To explore the relationship between peripheral arterial disease(PAD)and diabetic retinopathy(DR)in type 2 diabetes patients.[Methods]A total of 99 patients diagnosed with PAD were classified into grade 1-3 by their total scores of peripheral arterial stenosis assessed by color doppler ultrasound examinations, where the degree of stenosis 30% ~ 49% scored 0, 50%~99%scored 1,lumen occlusion(i.e. degree of stenosis 100%)scored 2,and therefore the total score 0-2 was categorized into Grade 1 ,3~4 into Grade 2 ,5~12 into Grade 3. The bilateral anterior tibial artery ,posterior tibial artery and dorsalis pedis artery of these patients were analyzed. The presence of diabetic retinopathy(DR)was graded from retinal photographs using a standard protocol.[Results]Among 99 cases of type 2 diabetic patients with peripheral arterial disease ,58.6%of them were male with average age of 67.3 ± 7.9 years old. Patients of Grade 1,Grade2,Grade 3 lesion accounted for 45.4%,30.3%,24.2%,respectively. Age, gender, smoking history,SBP,DBP,BMI,FBG,TC,TG,LDL-C,HDL-C,HbA1C among 3 groups were not statistically signifi-cant. The associations of DM duration and HbA1C value were significantly larger in DR than in PAD. The proportion of DR patients increased with the severity degree of PAD(p for trend=0.004). Degree of stenosis Grade 2 and Grade 3 could be predictive for DR.[Conclusions]DR is associated with the severity degree of PAD in type 2 diabetes patients as evaluated by duplex ultrasonography.Degree of stenosis Grade 2 and 3 could be used for screening or finding DR. Strategies for optimum treatment and early prevention are needed.
